Introdução: Este trabalho foi realizado com o objetivo de saber se houve agravamento das consequências do HIV após a percepção do estigma. Metodologia: Foi realizada uma revisão sistemática cujos critérios de inclusão foram (1) que os artigos tratassem de HIV / AIDS; (2) que eram artigos científicos; (3) que o idioma de publicação foi espanhol ou inglês. Os critérios de exclusão aplicados foram (1) que os artigos contemplassem a população infantil e adolescente; (2) que eram artigos de caso único. Resultados: A amostra total foi de 18.267 participantes, a maioria mulheres (65%), com média de idade de 39,3 anos, da África e da Ásia, casados ou com companheiro (a) estável e com baixo nível educacional e socioeconômico. O estigma foi analisado por meio da aplicação do teste em 55% dos artigos e da qualidade de vida em 44%. Foram encontradas repercussões psicológicas e sociais (53%) entre as quais se destacaram o transtorno depressivo e a redução da rede de apoio; trabalhista e econômica (21%); física (18%) e sexual (15%). Discussão: A maioria dos resultados mostrou que o estigma gerou repercussões, o que afetou, sobretudo, a saúde mental e a rede social desta população, o que refletiu a necessidade de promoção de apoio e tratamento psicológico. Da mesma forma, foi encontrada uma possível relação entre sexo, região e nível de escolaridade e o risco de contrair o HIV.
